OBSERVATION: Facility failed to pay annual operating fees for the year of 2021. ORDINANCE: Fees shall be paid once a year at the time of submittal of an inventory form. CORRECTIVE ACTION: Immediately pay the annual fee to the EHD Accounting Office located at 1868 E Hazelton Avenue, Stockton, CA 95205. Fee may also be paid online at https://www.sjgov.org/department/envhealth/fees (Convenience fee may apply). This is a Class II violation. 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. OBSERVATION: The business activities page and owner operator identification page have not been annually certified/submitted as a part of the business plan. The business activities page and owner operator identification page was not submitted for the years of 2018, 2019, 2020, and 2021. The business activities page and owner operator identification page has not been submitted since 6/20/2017. The business activities page and the owner operator identification page were submitted late on 1/30/2017 for the year of 2017. The Billing contact information, primary contact information, and the secondary contact information was all outdated on the business owner operator identification page. Updated information was submitted on 8/23/2021 for the primary contact, secondary contact, and for the billing contact. REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United States Code. An annual electronic submittal satisfies these requirements. The annual business plan shall be submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, review and submit/certify the current information. This violation has been corrected on site.
