PUBLIC HI!ALTH SERVICtS <br />SAN JOAQUIN COUNTY <br />ENVIRONMENTAL HEALTH DIVISION <br />Karen Furst, M.D., M.P.H., Health Officer <br />304 East Weber Avenue, Third Floor • Stockton, CA 95202 <br />209/468-3420 <br />PROPERTY ACCESS AUTHORIZATION <br />FOR <br />SOLID WASTE FACILITY/DISPOSAL SITE <br />INVESTIGATION AND ASSESSMENT <br />I hereby authorize California Integrated Waste Management Board (CIWMB) staff, their <br />designated contractors and representatives, and other affected State and local authorities access to <br />the property described below for the purpose of conducting a sub -surface site investigation to <br />determine the extent of the buried waste. The site investigation will include sampling the buried <br />waste material to further characterize the chemical and physical properties of the waste. The <br />investigation/sampling is warranted to identify potential hazards to public health and safety and <br />the environment, which may have resulted from previous solid waste disposal operations. This <br />investigation/sampling is being conducted pursuant to Public Resources Code (PRC) sections <br />44100 and 48020-48028, Solid Waste Disposal and Co -disposal Site Cleanup Program.
