Hazelton Avenue <br /> Stockton, CA 95205 <br /> County Waste Inspection March 26th 2019 <br /> Observations and Corrective Actions: <br /> #13 This registered large medical waste generator does not maintain individual treatment records <br /> and shipping and tracking documents for 2 years. A large medical waste generator required to <br /> register with the EHD shall maintain individual treatment operation records, and shipping and <br /> tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)- Log of the Sani I Pak alarms is not visible for review. Maintain the log of alarms at <br /> the facility for review by the EHD. <br /> CORRECTION: Implemented Alarm log effective immediately. Log was created and started on <br /> 03/26/2019. <br /> #24 Biohazardous waste is not placed in biohazard bag that is conspicuously labeled. Biohazardous <br /> waste shall be placed in a biohazard bag conspicuously labeled with words "Biohazardous Waste" <br /> or "BIOHAZARD". HSC 118275(a)(2)-The biohazardous container outside the soiled utility in the <br /> ER unit is missing biohazardous label. Make sure to have all biohazardous containers have the <br /> biohazardous label on them and visible. <br /> CORRECTION: Added stickers to containers; visible on top and sides. Monitor for continued <br /> compliance. <br /> #24 Nonradioactive and non-RCRA pharmaceutical wastes, which are regulated as medical waste, <br /> shall be placed in a container or secondary container labeled with the words. "HIGH HEAT OR <br /> INCINERATION ONLY" on the lid and sides, so as to be visible from any lateral direction. HSC <br /> 118275(a)(6)(B)- One of the pharmaceutical waste container in the Pharmacy is missing <br /> "Incinerator Only" label on it. Make sure that all pharmaceutical waste containers have <br /> "Incinerator Only" labels on all four sides and top of the container. Tubing was coming out of <br /> one of the pharmaceutical waste container in the ER unit (soiled utility). Ensure that nothing is <br /> coming out of the container. <br /> 525 W.
