The following is an itemized list of hazardous materials violations that have not been addressed for AutoZone #5684 as of December 01, 2021. Open violations from August 24, 2021 inspection Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. OBSERVATION: 1. Employee Adam Bloomer was last trained on 6/5/2020 and is past due for annual hazardous materials business plan training. 2. The following employees received their annual training late: -Gustavo Aceves received training on 5/3/2020, and received training late on 7/6/2021. - Isaiah Hinkle received training on 5/22/2020, and received training late on 8/5/2021. -Jose Gonzalez received training on 7/23/2020, and received training late on 7/26/2021. REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate notice,for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
