Respondents sha11 make an inal payment of <br />$1,109.60 within 30 days of the effective date of this consent order. <br />Subsequent payments of $1750.00 shall be due February 6, 2020, and April <br />2020, for a total of $4,609.60. All additional CUPA costs will be billed at the <br />current hourly rate. Respondent's checks shall be made payable to "San <br />Joaquin County Environmental Health ®epariment," and shall be delivered <br />together with the attached Payment Voucher to: <br />San Joaquin County Environmental Health ®epartment <br />Certified Unified Program CUPA <br />186 E. Hazelton Avenue <br />Stockton, California 95205 <br />A photocopy of the check shall be sent to the following CUPA personnel <br />assigned to this matter, Michelle Henry, San Joaquin County Environmental <br />Health ®epartment <br />If Respondents fail to make payments as provided above, Respondents agrees <br />to pay interest at the rate established pursuant to HSC § 25360.1 and to pay all <br />costs incurred by the CUPA in pursuing collection including attorney's fees. <br />OTHER PROVISIONS <br />11.
