S0 JOA U 1 Environmental Health Department <br /> U ;� i Y I <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FARWEST SAFETY INC 226 N MAIN ST, LODI September 29, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br /> OBSERVATION: The Business Owner/Operator Identification form was not submitted for 2019 and 2020. <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: This violation was corrected with the 01/08/2021 CERS submittal. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The hazardous materials inventory was not submitted for 2019 and 2020. <br /> REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br /> the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code.An annual electronic submittal satisfies these requirements.The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> CORRECTIVE ACTION: This violation was corrected with the 01/08/2021 CERS submittal. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION:A site map was not completed and submitted as part of the business plan for 2019 and 2020. The <br /> site map is incomplete,the following required elements are missing: <br /> -loading areas <br /> -hydraulic oil storage area <br /> -emergency shut-offs <br /> -all access/exit points for all structures <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment.
