The site <br /> map has not been submitted since 06/19/2017. The site map was missing the following required items: <br /> -loading areas <br /> -internal roads <br /> -adjacent business/buildings <br /> -storm drain observed in the blue shop <br /> -storage areas for argon, carbon dioxide, oxygen, and acetylene <br /> -emergency shut-offs <br /> -all access/exit points: driveway/gate was missing on the map <br /> REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br /> the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code.An annual electronic submittal satisfies these requirements.The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response plan has not been annually certified/submitted as a part of the business <br /> plan. The response plan has not been submitted since 06/19/2017.
