<br /> OBSERVATION: Facility failed to inspect each aboveground container for integrity based on industry standards, as
<br /> discussed in the SPCC Plan on a regular schedule. Per the SPCC plan,the facility is utilizing industry standard STI
<br /> SP001. The following observations were made during the inspection:
<br /> -All aboveground containers are not inspected monthly, the last monthly inspection was performed on 06/07/2021.
<br /> -The 01/04/2021 monthly above ground tank inspection was not completely filled out.
<br /> -Per the SPCC plan,the 10 thousand gallon diesel tank was due for a formal external inspection in 2010. During
<br /> the inspection, EHS Specialist Gloria Suarez could not confirm that a formal external inspection for the 10 thousand
<br /> gallon tank was ever conducted and the facility did not have any records.
<br /> REGULATION GUIDANCE: REGULATION GUIDANCE: 112.8(c)(6)Test or inspect each aboveground container
<br /> for integrity on a regular schedule and whenever you make material repairs.You must determine, in accordance
<br /> with industry standards,the appropriate qualifications for personnel performing tests and inspections,the frequency
<br /> and type of testing and inspections,which take into account container size, configuration, and design (such as
<br /> containers that are: shop-built,field-erected, skid-mounted, elevated, equipped with a liner, double-walled, or
<br /> partially buried). Examples of these integrity tests include, but are not limited to: visual inspection, hydrostatic
<br /> testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive
<br /> testing.You must keep comparison records and you must also inspect the container's supports and foundations. In
<br /> addition, you must frequently inspect the outside of the container for signs of deterioration,discharges,or
<br /> accumulation of oil inside diked areas. Records of inspections and tests kept under usual and customary business
<br /> practices satisfy the recordkeeping requirements of this paragraph.
<br /> 112.7(e) Inspections,tests, and records. Conduct inspections and tests required by this part in accordance with
<br /> written procedures that you or the certifying engineer develop for the facility. You must keep these written
<br /> procedures and a record of the inspections and tests, signed by the appropriate supervisor or inspector,with the
<br /> SPCC Plan for a period of three years. Records of inspections and tests kept under usual and customary business
<br /> practices will suffice for purposes of this paragraph.
<br /> CORRECTIVE ACTION: Immediately conduct the necessary inspections for all aboveground tanks based on
<br /> industry standards, on a regular schedule, after making material repairs, using non-destructive testing, inspecting
<br /> each container's supports,foundations, and outside for signs of deterioration, discharges, or accumulation of oil
<br /> inside diked areas. Maintain testing and inspection records. Submit a copy of the inspection results to the EHD.
<br /> This is a repeat violation, Class II.
<br /> Overall Inspection Comments:
<br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was
<br /> finished on 10/20/2021 and replaces the initial checklist.
<br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
<br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting
<br /> paperwork, within 30 days of receiving the complete inspection report.
<br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead
<br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate
