Examples of these integrity tests include, but are not limited to: visual inspection, hydrostatic
testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive
testing. You must keep comparison records and you must also inspect the container's supports and foundations. Maintain testing and inspection records. Submit a copy of the inspection results to the EHD.
This is a repeat violation, Class II.
Overall Inspection Comments:
An inspection checklist was provided to the facility operator on the day of inspection. A complete report was
finished on 10/20/2021 and replaces the initial checklist.
Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
documenting the corrective actions that have been or will be taken for each violation, and any supporting
paperwork, within 30 days of receiving the complete inspection report.
To minimize person to person contact EHD is choosing to write the name of person receiving the report instead
of having them sign.
