SAN JOAQUIN <br /> Environmental Health Department <br />AMENDED <br />2/10/2021 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />MANTECA DENTAL CARE <br />Facility Address: <br />1007 S MAIN ST, MANTECA <br />Date: <br />February 09, 2021 <br />An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br />finished on February 10, 2021 and replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />An EPA ID number will be needed if the facility generates hazardous waste other than universal waste - dental <br />amalgam and photo fixer being recycled for silver content. Go here to apply for an ID number: <br />https://dtsc.ca.gov/apply-for-hazardous-waste-epa-id-number/ . <br />Documents provided during inspection: Modified contingency plan. <br />Waste streams found: ImageMax Developer Cleaner, ImageMax Developer, ImageMax Fixer, <br />mercury-containing dental amalgam, universal waste electronic devices, universal waste batteries. Empty <br />nitrous compressed gas cylinders are returned to the gas vendor. <br />The facility completed hazardous waste generator notification on the California Environmental Reporting System <br />and completed the modified contingency plan during the inspection. <br />Robert McClellon and Sandip Singh of the EHD were on-site at the same time, conducting the Medical Waste <br />Management Act (MVVMA) inspection. <br />Wastes generated by the facility that are covered by the MVVMA: Anesthetics such as Articaine, Sedation pills <br />such as Valium, and items contaminated with blood or bodily fluids. <br />Mary Tran stated that dental tool sterilization is done using steam.
