HSC117945 — Shipping/ tracking records were not available for review at the office. Tracking records should <br />be maintained at the facility for 3 years. Provide EHD with records from April 2019 to current date. <br />24. Biohazardous waste is not placed in a biohazard bag that is conspicuously labeled. <br />Biohazardous waste shall be placed in a biohazard bag conspicuously labeled with words "Biohazardous Waste" or <br />"BIOHAZARD". HSC 118275(a)(2) - The area above the medical waste collection cabinet is labeled with the international <br />biohazard symbol. However, the medical waste collection cabinet should be conspicuously labeled rather than the area <br />above it. Ensure the cabinet door is labeled with the international biohazard symbol. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately labeled. <br />Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) — The biohazard bag in the storage area was not <br />contained in a rigid container. Place biohazard bags in a rigid container for storage. <br />25. Biohazardous waste is stored above 32 F onsite for days. <br />- A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br />onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br />store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) — Biohazardous waste is hauled by Stericycle on <br />a quarterly basis (every 3 months). Increase the frequency of biohazardous waste pickup to every 7 days if the amount <br />of waste generated is 20 or more pounds per month or increase frequency of pickup to every 30 days for less than 20 <br />pounds of waste generated per month. <br />Please have all violations corrected prior to the reinspection. A reinspection will be <br />conducted on March 10, 2021, there will be a reinspection fee of $152 per hr. <br />NOTES: <br />Stericycle is the facilities medical waste hauler. <br />Pharmaceutical waste, sharps waste and biohazardous waste are picked up by Stericycle every 3 months. <br />Reinspecion on/about: .
