OBSERVATION: The following were not attached to the designated operator inspection reports: -alarm history printout for 6/14/21 -work order for the March 2021 UDC 7/8 sensor replacement -list of alarms (including 91 STP sump 12/22/20, dispenser 7/8 3/22/21, and dispenser 5/6 3/27/21) REGULATION GUIDANCE: The designated operator visual inspection report shall include: -a copy of documentation demonstrating action taken in response to each compliance issue identified during the previous inspection -a copy of the alarm history since the previous inspection -a copy of documentation demonstrating action taken in response to each alarm since the previous inspection -a list of each alarm since the previous inspection for which there is no documentation of the alarm condition and action taken in response CORRECTIVE ACTION: Ensure all required documents are attached to future designated operator inspection reports. This is a repeat violation, Class Il. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. OBSERVATION: A single Facility Employee Training Certificate was incorrectly used for multiple dates, in August 2020 and August 2021. The certificate had no CERS ID, no dates of assuming responsibility as a facility employee, and incorrect initial training dates. REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a Class II violation.
