Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />October 29, 2021 <br /> Facility Address: <br /> 8228 West Ln, Stockton <br /> Facility Name: <br /> AutoZone #5685 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The following elements are missing: <br />-Please define all abbreviations on site map <br />-spill kits <br />-loading areas <br />-internal roads <br />-storm and sewer drains <br />-one fire extinguisher was observed near the southeast corner of the store and was not included on the site map <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. <br />-2019 submitted late on 1/18/2019 <br />-2021 submitted late on 1/27/2021 <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: This was corrected on 1/27/2021. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. Complete and submit a copy of the Return <br />to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been <br />or will be taken for each violation, and any supporting paperwork, by November 29, 2021. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152).
