The site map is <br /> incomplete. <br /> -Fire extinguishers were found between the dispensers and emergency shutoff and were not listed on the map. <br /> -The storm and sewer drains located near/by the dispensers were not listed on the map. <br /> -Internal roads were not listed on the map. <br /> -The propane is not listed on the map. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. <br /> -The 2019 HMBP was submitted on time on 12/11/2018 but was incomplete. <br /> -The 2020 HMBP was not submitted. <br /> -The 2021 HMBP was submitted on time on11/9/2020 but was incomplete. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> FA0023733 PR0542361 SCO01 10/13/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 4 Hazardous Materials Program OIR <br /> 1868 E.
