Main Street, Mountain House, CA 95391 Tel (209) 831-2300 • Fax (209) 831-5610 www.mountainhousecsd.org Community Development Department 1810 East Hazelton Ave Stockton, CA 95205 January 3, 2022 Re: Application Referral PA-2100171 Dear Giuseppe: Submitted Electronically The Mountain House Community Services District (CSD) has reviewed application referral documents for PA-2100171, which was transmitted electronically on December 22, 2021. MHCSD has the following comments, questions and/ or concerns: 1. Is the applicant proposing to use existing conduits to install the fiber optic lines? If not MHCSD will need to review plans on how this conduit will be installed. 2. Are any improvements being made to the existing driveway? 3. The applicant shall pull an encroachment permit with MHCSD before starting work. 4. The applicant shall submit a truck hauling route plan for MHCSD's approval before staring work. 5. The applicant shall submit a traffic control plan for MHCSD's approval before starting work. 6. Any improvement within the Mountain House CSD right of way shall be reviewed and approved by Mountain House Community Services District. Any damage done to existing improvements shall be repaired to MHCSD's satisfaction. 7. The site is outside of Mountain House Community Services District and the CSD does not expect any utility lines for this project (Water, Stormwater, and Sewer) to be connected to the MHCSD utility lines.
