Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />December 20,2021 <br />Facility Address: <br />2540 TEEPEE DR, STOCKTON <br />Facility Name: <br />SLAKEY BROTHERS 10 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />RemarksItem # <br />5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for Freon, TU PT1-1, TU MT-1 PRO and Travel Tack <br />were not complete/accurate. <br />1.Freon (R22, R421A, and R410A) –the material is reported in gallons vs. pounds. <br />2.TU PT-1 and TU MT-1 Pro (propane and propylene) –the material is reported in gallons vs. cubic feet <br />3.Travel Tack (Methyl Acetate, Hexane) –the physical state is reported as gas vs. liquid. <br />REGULATION GUIDANCE: The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map uploaded at the time of inspection is not complete. The site map does not <br />show/address the following required elements: <br />1.Loading areas. <br />2.Internal roads. <br />3.Access and exit point (gate) <br />4.Emergency response equipment (spill control). <br />5.Emergency shutoffs. <br />6.The locations of the R-421A, R410A, R22, adhesive, TU PT-1, TU MT-1 Pro, Coil Cleaners, muriatic acid, <br />travel tack, propane. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />FA0010874 PR0520527 SC001 12/20/2021 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
