Apply pesticides covered by SLN(s) and Section 18(s) in conformance with the SLN(s) and Section 18(s). The SLN and/or <br />Section 18 must be present at the use site along with the registered label. <br />4. A flagger is required at the application site for all AIR applications of Zinc Phosphide bait. <br />5. The applicator shall submit Notice of Intents (NOls) for any California Restricted Material to be used. NOls will only be <br />accepted electronically (CalAgPermits), in-person, or over the phone. NO NOI will be accepted by email or fax. NOls, in any <br />submitted form, for weekend applications will NOT be accepted after 12:00pm Friday. Any California Restricted Material <br />application proposed before 12:00pm Monday must have the corresponding NOI submitted no later than 4:00pm the prior <br />Friday. EPA Registration Numbers for the proposed material to be used should be submitted on the NOI form. <br />6. For commodity fumigations, the term "Direct Supervision" is defined at minimum as a qualified applicator (holding a <br />Department of Pesticide Regulation issued Qualified Applicator's Certificate (QAC) or Qualified Applicator's License (QAL) <br />card, as the situation demands, in the subcategory "A") who is physically on-site during the application and initial aeration of <br />the chamber. <br />SITE CONDITIONS <br />Appendix H Aluminum Phosphide and Magnes <br />Aluminum Phosphide and Magnesium Phosphide for Burrowing Rodents <br />See attached: Appendix H <br />QL DICAMBA AND PHENOXY HERBICIDES <br />CONDITION - QL - DICAMBA AND PHENOXY HERBICIDES - North of Eight Mile Road <br />In addition to all applicable laws, regulations and label restrictions, the CONDITIONS BELOW APPLY to applications covered <br />by this permit. <br />1. No ester formulation applications allowed. <br />2. Do not discharge material more than 10 feet above the crop or target. <br />3. A drift reducing agent shall be added to the spray mixture at label rates. <br />4. Maximum wind velocity for Phenoxy herbicides is 10 m.p.h. <br />5. Maximum wind velocity for Dicamba is 10 m.p.h. EXCEPT when wind is moving in direction of sensitive crops, then the <br />maximum wind velocity is 5 m.p.h. <br />6. No Dicamba or Phenoxy herbicide applications are allowed beginning March 16 and continuing through October 15, or <br />longer until grape leaves in the vicinity of the treated area are "browned off'. <br />
