SAN„JOAQUIN • <br />Ls!k-d: —COUNTY— <br />Crtotntss 9ro6,1 heft!' <br />Environmental Health Department <br />WASTE DISCHARGE/SPILL RESPONSE <br />NOTICE TO ABATE <br />Responsible Party: PG&E Date: Apr 16, 2020 <br />RP Mailing Address: PO BOX 770000 <br /> Record ID#: C00051856 <br />SAN FRANSISCO, CA 94177- Program Element: 2546 <br />Spill Location: 9015 W WALNUT GROVE RD, THORNTON <br /> Release/Spill Response (excluding Joint Team) <br /> <br />NATURE OF WASTE DISCHARGE/WASTE RELEASE: <br />APPROXIMATELY 20 GALLONS MINERAL OIL RELEASED FROM A PAD-MOUNTED TRANSFORMER. <br />OBSERVATIONS: <br />Cleanup was conducted by Dave Larsen (PG&E). The non-PCB mineral oil on the transformer pad was cleaned using with <br />absorbent and penetone. Dave determined additional cleaning was needed on surrounding soil, so PG&E initiated a USA <br />dig notification. The impacted area was coned off pending USA dig clearance for removal of impacted soil. <br />Please be aware that all impacted soil has to be excavated and disposed of under the appropriate waste manifesting <br />procedure [HSC 25160 (b)(3)]. The disposal of any hazardous waste, or the causing thereof, is prohibited when the disposal <br />is at a facility which is not permitted or authorized for hazardous waste disposal. [HSC 25189.5 (a).] Notify EHD (48-hours) in <br />advance of any cleanup activities. Clean up all spilled material and dispose of it under all appropriate waste manifesting and <br />waste disposal laws and regulations. Provide waste determination and documentation for all generated waste. <br />VIOLATIONS: <br />Discharge of any waste, including sewage <br />(CA Health and Safety Code 5411) <br />Discharge of waste or sewage to water or within 150 ft. of waterway <br />(SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br />Improper disposal of a hazardous waste <br />(CA Health and Safety Code 25/89.5) <br />Permitting or Maintaining a Nuisance <br />(SJC Ordinance Code Section 8-5100) <br />Other Violation <br />CORRECTIVE ACTIONS / ORDER: <br />Investigate and abate contamination under the oversite of the appropriate lead agency. <br />Submit copies of hazardous waste manifests/disposal records to the EHD. <br />Other Correction <br />WASTE DISCHARGE/SPILL RESPONSE NOTICE TO ABATE EHD 25-01 Rev 08/17/17 Page 1 of 2 <br />1868 E.
