Changes to your Business Owner/Operator <br />Identification Page, Hazardous Materials Management Plan, facility map, or chemical description <br />pages must be reported to our office within 30 days. <br />Once a year, businesses are required to certify to our office that their HMMP information is current and <br />accurate. The easiest way to meet this requirement is through our HMMP Compliance Website: www. <br />sjoesdata.org. Please log onto the site and review your HMMP and chemical inventory and make <br />necessary changes. When you are satisfied that all information is correct, return to the Main Menu and <br />click on the link entitled "Office of Emergency Services Certification Form." Read the Certification <br />Statement that appears, check the box below the statement, and click on the "Submit Certification" <br />button. As a.reminder, the website username and password for the facility shown above is as follows and <br />should be kept confidential: <br />Username: Password: <br />If you do not have access to the Internet, please review and update your hard copy documents. Contact <br />our office for a blank Certification Statement and forms. Please complete, sign, and date the Certification <br />Statement and return it with any updated documents to our office by the deadline shown below. <br />You are required to retain a current and accurate copy of your Hazardous Materials Management Plan at <br />your facility. If you have any questions, please contact our office at (209) 953-6200. Your business must <br />have its Certification Statement on file with our office no later than January 29, 2009 Your <br />response is required by law.
