The owner/operator shall maintain the <br />following documents and records at the facility: <br />a) the job title for each position at the facility related to hazardous waste management, and the name of each employee filling <br />each job,- <br />ob;b) <br />b)written job description for each position listed in a) above, <br />c) written description of the type and amount of both introductory and continuing training that will be given to each person <br />filing positions listed in a) above, <br />d) records that document that the training has been given to and completed by facility personnel. <br />Training records on current personnel shall be kept until closure of the facility. Training records on former employees shall be <br />kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial <br />or refresher hazardous waste training. Within 30 days (July 11, 2011) provide this office with a copy of the roster and the <br />syllabus which meet the requirements of 66265.16 CCR (California Code of Regulations). <br />60, 61, & 62. Manifest number 005525939JJK was found without a signed off copy from the destination facility. Hazardous <br />waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have <br />them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br />owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was <br />accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated <br />facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest <br />within 45 days of the date that the waste was accepted by the initial transporter. the generator shall submit an Exception <br />Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to <br />locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC.
