On February 21, 2013, a permit application, SR0066649, was submitted retroactively for <br />the ISD upgrade performed on August 3, 2012. Mr. Cacapit approved the permit on <br />March 5, 2013 (Attachment 63). <br />On February 22, 2013, Ms. Stacy Rivera, Senior REHS, EHD, witnessed the successful <br />testing of the 87 -octane leak detector and spill container, and 87 -octane siphon and <br />diesel UST annular sensors. The 91 -octane spill container was also tested, but failed. <br />Ms. Rivera removed the red tags on the 87 -octane tanks (Attachment 64). <br />On April 1, 2013, Mr. Cacapit sent a follow up letter to the September 4, 2012, and <br />September 28, 2012, UST inspections (Attachment 65). <br />Attachments 66, 67, 68, 69, and 70 include parcel information, facility information, tank <br />owner information, UST facility file narrative, and hazardous waste file narrative, <br />respectively. <br />II.
