The facility generated 3 lbs of RCRA acute hazardous waste in <br /> June 2017(see IMG 1986-1987)and 6 lbs of RCRA acute hazardous waste in September 2017(see IMG <br /> 1980-1981). <br /> A Biennial Report, USEPA Form#8700, shall be prepared and submitted to DTSC by March 1 or each <br /> even-numbered year and maintained on site for at least three years. Immediately prepare a biennial report,submit <br /> it to DTSC(and a copy to the EHD)by 11/1/2018, 30 days from now, and maintain it on site. <br /> This is a Class II violation. <br /> 109 CCR 66265.52 Contingency plan is incomplete. <br /> -The facility's contingency plan is incomplete.The facility failed to designate a list of names, addresses, and phone <br /> numbers(office/home/cell)of all persons qualified to act as emergency coordinator, on the contingency plan.Where <br /> more than one person is listed, one shall be named as the primary emergency coordinator and all others in order in <br /> which they will assume responsibilities. <br /> -The contingency plan also lacks the current telephone number for the State Office of Emergency Services. <br /> The facility's contingency plan is incomplete.The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires,explosions, or any sudden or <br /> non-sudden release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names,addresses, and phone numbers(office/home/cell)of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br /> all others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment,which includes location, physical description,and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan,the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br /> fulfill the requirements of this chapter.
