All facility communications or alarm systems,fire protection <br /> equipment, spill control equipment, and decontamination equipment,where required, shall be tested and maintained <br /> as necessary to assure its proper operation in time of emergency. Immediately begin this practice. <br /> This is a Class II violation. <br /> 403 CCR 66265.173 Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> -In the store's HW storage area, a Bubble Mate Foot Spa was found with a Toxic HW label, however the item, <br /> including the item's box,was not contained in a HW tote(IMG 2049-2050). The boxed item did not appear to fit into <br /> the totes CVS uses for its HW. <br /> The item listed above was identified as waste by Mary Young at the time of inspection. <br /> -In the pharmacy, an open blue bin labeled with"Weekly StrongPak Accumulation Bin,""Capped Amber Vials(bag <br /> liquids),"and"Loose Pills(bagged)"was found containing bags of loose pills and vials with pills(IMG 2060, <br /> 2062-2063). According to Trixia, loose pills and other pharmaceuticals that are to be managed as HW are first <br /> placed in this blue bin until processed and put into the pharmacy HW tote every Friday, at the latest. <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste.
