Test last year was completed end of July 2012. Due to conflicts the <br />tests could not be completed due to scheduling conflicts but was completed within 30 days of the test <br />the prior year. <br />110: Leak test performed on July Bch <br />111: Spill containment testing performed on July 81h. <br />117: All work to the system is done by licensed third party contractors who are responsible for obtaining <br />permits. To our knowledge all permits to repair have been obtain for all work done to the system known <br />us. We had no knowledge of a battery replacement or any reset to the monitoring panels. Truck side <br />testing was last completed on July 30, 2012, after the reprogramming is suspected to have been done <br />and all was reported working correctly. A copy of that test was provided'to the EHD in 2012. <br />204: We record all alarms on site separate from the designated operator. We have alerted the DO to the <br />missing information and have asked him to make sure they are recording correctly in the future. <br />206: DO is responsible for maintaining required by logs. We have advised them of their oversight. <br />311: Permit was obtained for the work and was corrected per the EHD. <br />402: A small amount of liquid was found in the STP sumps. Investigation has been started into the <br />source of the diesel but have no found and actively leaking components. There may have been reports <br />of diesel in the dispensers but all the dispensers at the time of inspection had no diesel in the pans. If <br />any issues were reported in the past they were corrected immediately. The sites had in the past issues <br />with people breaking into the dispensers and breaking certain components within the dispenser. At any <br />point this may have occurred a licensed contractor made the repairs. <br />Return to Compliance: Hazardous Waste Inspection <br />804: Lamps were in a container stored on site. The container has sincebeen covered per the request of <br />the EHD. <br />808: There were two boxes on site on with a label and another one right next to it without a label. The <br />box was labeled on site. <br />135: A report was submitted last year but the EHD did not report not receiving the information until July <br />9, 2013. <br />
