Failed to store universal waste lamps in a closed container and in a manner that prevents breakage <br />and release to the environment. Approximately 18 four foot waste fluorescent tubes are stored vertically <br />inside an open cardboard box located in the back storage room. Lamps shall be packaged in a container <br />that is structurally sound, adequate to prevent breakage and compatible with the contents of the lamp. It <br />must be remained close to prevent leakage, spillage or damage. Correct by storing the waste fluorescent <br />tubes in a container that would fully enclose the tubes and can be kept closed. Submit a photograph of <br />the new storage container for universal waste fluorescent tubes. If proper container can not be found, <br />have a hazardous waste hauler remove the fluorescent tubes to an authorized hazardous waste facility <br />and provide a copy of disposal receipt to EHD. Ensure all future fluorescent tubes are properly stored in <br />808. Failed to label universal waste to identify type of universal waste. Two cardboard boxes storing <br />waste fluorescent tubes have incomplete labeling. The Phillips cardboard box was labeled "Universal <br />Bulbs" and date of 12-15-2012". The other open cardboard box did not have any label or identification of <br />type of waste. Universal waste shall be labeled or marked "Universal Waste" along with type of universal <br />waste (Battery, Electronic Devices, CRTs Glass) ie "Universal Waste - bulbs/ lamps". Ensure all future <br />storage containers of universal waste is properly labeled. <br />- Corrected on site.
