Ashley stated that she couldn't <br /> remember a time when the pharmacy ever had expired warfarin, since it is rapidly used up by consumers. <br /> However, Ashley retrieved a bottle of un-expired warfarin (see photo IMG 1573)from a pharmacy shelf and <br /> processed the bottle of warfarin into the pharmacy computer system as expired. Ashley revealed that the <br /> computer system prompted her to return the bottle of warfarin to Genco(IMG 1572). Please note that if the <br /> facility were to return expired warfarin or coumadin to Genco, the facility would be in violation of Health and <br /> Safety Code(HSC) 25163(a)for transporting or transferring hazardous waste (HW) to a transporter without <br /> valid registration (Violation#201) and HSC 25189.5(a)for disposing or causing disposal of HW at an <br /> unauthorized point(Violation#202). <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has amended <br /> and written the complete report which replaces the initial checklist, on 9/13/2018. Violation#105 was removed. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
