Juana
stated that the date on the label was incorrectly written and should have read 7/21/18. Juana corrected the date on
the label to 7/21/18 at the time of inspection.
- A HW Corrosive Bases tote with accumulation start date 8/2/18 (IMG 1914), contained Mio Fruit Punch Liquid
Water Enhancer, which was marked as and scanned as Corrosive Acid HW (IMG 1915-1917). Juana replaced the
Corrosive Bases label with a HW Corrosives Acids label (IMG 1921) at the time of inspection.
- CVS Health Hydrogen Peroxide was found leaking in a StrongPak bag without absorbent or clean-up debris inside
an Oxidizers HW tote (IMG 1918-1920). When asked what happened to the absorbent or clean-up debris used to
clean-up the leakage, Juana did not know. Juana added absorbent into the StrongPak bag to absorb the leakage,
and double bagged the item at the time of inspection (IMG 1922-1923).
- Meguiar's Cleaner Wax liquid (for automobiles) was found leaking in two StrongPak bags without absorbent or
clean-up debris inside a Toxic HW tote (IMG 1924-1927). When asked what happened to the absorbent or clean-up
debris used to clean-up the leakage, Juana did not know. Juana added absorbent into the StrongPak bags to
absorb the leakage, and placed the bagged item into a third bag at the time of inspection (IMG 1929).
- A Toxic HW tote with accumulation start date 8/31/18 lacked the physical state on the label (IMG 1927). Juana
Hernandez marked the physical state on the label at the time of inspection (IMG 1928).
- CVS Pharmacy MAX Hearing Aid Batteries Size 312 was found marked as "TX" and inside a Toxics HW tote with
accumulation start date 8/31/18 (IMG 1928, 1930-1931). This item scanned as Toxic HW (IMG 1932). When asked
why this item was placed in a Toxics HW tote, Juana stated because that is where the scanner prompted the
employees to place the item. When asked what employees do if they are unsure of an item's hazardous waste
categorization, Juana stated they follow what the scanner prompts them to do. Juana did not know to call the CVS
Safety Hotline or the CVS Environmental Contact for questions regarding hazardous waste categorization.
- A partially empty container of Wet Line Extreme Professional Styling Gel was found in an open black basket, which
was placed inside an open clear container labeled as Vendor Returns (IMG 1933-1936). The item scanned as Toxic
HW. Juana identified the item as waste. Juana bagged the item and placed it inside a Toxic HW tote at the time of
inspection.
- Juana stated that returned and damaged items are brought to the facility's back storage area where they wait in
open red totes to be processed. When asked how often these items are processed, Juana stated that these items
are processed on a weekly basis, rather than daily, due to the recent remodeling of the store. The following was
found in an open unlabeled red tote on a shelf adjacent the HW storage area (IMG 1960):
- Gold Bond Medicated Extra Strength Lotion scanned as Toxic HW (IMG 1941-1943)
- L.A. Girl Eyeshadow Collection ULTRA scanned as Toxic HW (IMG 1944-1946)
- L.A. Girl Liquid Lipstick scanned as Toxic HW (IMG 1951-1953)
- Two Wet N Wild Megaslicks Lip Gloss scanned as Toxic HW (IMG 1954-1959)
The items listed above were identified as waste by Juana.
FA0003286 PR0535759 SCO01 09/21/2018
EHD 22-03 Rev. 09/22/16 Page 8 of 12 RCRA Large Quantity Hazardous Waste Generator OIR
