The missing alarms include: • L14 Waste Oil Annular-fuel alarm on 4/21/2021 • L15 Hi Limit-fuel alarm on 4/21/2021 REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. --------------------------------------- --------------------------------------- 2)OBSERVATION:The designated operator failed to check and note the correct test dates on the designated operator inspection reports from April 2021 through December 2021. The following test dates were incorrect on designated operator inspection reports from April 2021 through December 2021. •The correct date for the last Monitoring System Certification Testing and Spill Container is 4/21/2021 but was listed as'1/27/2021'on these designated operator inspection reports. •The correct date for the last Overfill Prevention Equipment Inspection is 1/31/2018 but was listed as'not applicable'on these designated operator inspection reports. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. --------------------------------------- --------------------------------------- 3)OBSERVATION: Designated operator reports were not signed within the required time frame.The designated operator reports from April 2021 through December 2021 were not signed by the owner/operator. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class Il.
