Page 3 of 5 <br />Facility Number: <br />SOLID WASTE FACILITY PERMIT <br />39 -AA -0024 <br />16. Self Monitoring: <br />The owner/operator shall submit the results of all self monitoring programs to the Enforcement Agency within 30 days of the end <br />of the reporting period (for example, P quarter = January — March, the report is due by April 30, etc:. Information required on <br />an annual basis shall be submitted with the 4`hquarter monitoring report, unless otherwise stated. <br />Program <br />Reporting Frequency <br />a. Log of special occurrences (i.e. fires, explosions, accidents, inadvertent acceptance of <br />Keep on site and available upon <br />hazardous wastes, etc.) must be kept and made available upon request. <br />request of LEA <br />b. Samples shall be analyzed for <br />request of LEA <br />constituents required per 14CCR, Section 17868.1. <br />e. Pathogen reduction, including time and temperature of compost, shall be kept on site and <br />Keep on site and available upon <br />made available upon request (14CCR, Section 17868.3) <br />request of LEA <br />f. The number and types of vehicles using the facility per day must be kept on site and made <br />Quarterly <br />available upon request. <br />g. Records showing the results of the hazardous waste load checking program, including <br />Onsite and available upon <br />quantities and types of hazardous wastes, medical wastes or otherwise prohibited waste <br />request of LEA <br />found in the waste stream and disposition of these materials shall be kept on site and made <br />available <br />h.
