The following is an itemized list of hazardous waste violations that have not been <br /> addressed for SALLY BEAUTY SUPPLY as of June 02, 2020. <br /> Open violations from December 14, 2018 inspection <br /> that the waste was last disposed of. <br /> Immediately locate a copy of the hazardous waste determination made for any waste not already listed on the <br /> "Hazardous Materials Management" poster or in the Hazardous Waste Training Guide, ensure store employees are <br /> familiar with these waste determinations, and submit a copy to the EHD. If a copy is unavailable, immediately make <br /> a hazardous waste determination and begin managing the above listed waste under Title 22 hazardous waste <br /> regulations. <br /> ❑x This violation was corrected ❑This violation will be corrected by(date): <br /> © Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> In accordance with CCR 66262.11 Subsection (b), SBH relies upon generator knowledge and training to make waste <br /> determinations based on general product classes. Subsequent to your inspections, SBH updated its training materials <br /> which, among other hazardous-waste related matters, included a supplement exclusively on waste determination. A <br /> copy of the updated training material was provided to you. <br /> Generator knowledge continues to be the waste-determination methodology utilized by SBH, however, in the spirit of <br /> returning the Sally Beauty Supply and CosmoProf stores in San Joaquin County to compliance, attached is a <br /> spreadsheet of waste-determinations for the products identified in your inspection reports as being incorrectly <br /> categorized.
