Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) handling or the training provided has been inadequate. According to Brittney Kaufman, the store manager, all employees have been trained to place damaged or returned items into the HW accumulation totes.An Employee Hazardous Waste Training Log was found in the Sally Beauty Holdings, Inc. Hazardous Waste Training Guide Binder, and had the signatures for Brittney K., Gaby C., Kiana G., Elliz M., and Nadira P. dated 12/1/18 (see photo IMG 3663, 3665). The following observations were made in the store's back storage room at the time of inspection: -The most recent Weekly Inspection Log was completed on 1/8/19 by Brittney and indicated all containers were in good condition (IMG 3667). However the following HW totes were found with cracked/broken lids: -Corrosive Acids HW tote(IMG 3674-3677) -Corrosive Bases HW tote(IMG 3678-3680) -State Toxic HW tote(IMG 3712, 3714) Brittney acknowledged the lids were broken, but indicated she thought the lids were still okay to be used. -A Roux Fanci-Full Instant Hair Color in Hidden Honey color, which according to the label"lasts until next shampoo,"was found in the Flammables HW tote (IMG 3681, 3688).According to Brittney, this item is a temporary liquid hair color.At the time of inspection, Brittney could not access the Safety Data Sheet(SDS)for this item via the Sally Beauty SDS application on the store's iPad(IMG 3689). Brittney stated the item was placed in the Flammables HW tote since"Fanci-full mousse and liquid"is listed on the Hazardous Waste Information Guide posted above the HW tote storage area, and liquid permanent hair colors should be placed in the Flammables HW tote(IMG 3682-3683, 3685). However,the SDS for this item,which was found after the inspection via the Online SDS System -Sally Beauty Supply Accutrack/ORC, states the item is not flammable.The Sally Beauty Holdings, Inc. Hazardous Material Management poster(The Poster),which was also posted above the HW storage area, states non-flammable semi-permanent and temporary hair color should be placed in the State Toxic HW tote(IMG 3686). -Two containers of ProDip by Supernail dip powder, containing benzoyl peroxide,were found in the Flammables HW tote(IMG 3704-3706).According to the SDS for these items,these items are not flammable (IMG 3707-3711). According to The Poster, items containing peroxides should be placed in the Oxidizers HW tote.Also, according to a Sally Beauty training video called How to Categorize Products(California),found on the Athena application on the store's iPad, a similar item called ASP Bonding Acrylic Powder also containing benzoyl peroxide, is to be placed in the Oxidizers HW tote. Brittney placed the two containers of ProDip dip powders into the Oxidizers HW tote. -A red bottle of Proclaim Glossing Polish Color and Heat Protection was found in the State Toxic HW tote(IMG 3719-3721). The SDS for the item states the item is not flammable (IMG 3722-3723), however the back label on the item stated the item was flammable. Brittney placed the item in the Flammables HW tote. -The Oxidizers HW tote label indicated"bleach containing cleaning products, laundry bleach, and oxidizing cleaners"as the contents of the tote(IMG 3730), however the following items were inside the tote at the time of inspection: -An Ion 10 Volume Developer(IMG 3731) -An One'N Only Argan Oil Hair Color Cream Developer(IMG 3732) -Two containers of ProDip by SuperNail dip powders(IMG 3733) -The Corrosive Bases HW tote label indicated"dishwasher detergents, drain cleaners,floor strippers, glass FA0023426 PR0540939 SCO01 01/15/2019 EHD 22-01 Rev.09/22/16 Page 5 of 8 Small Quantity Hazardous Waste Generator OIR
