Hanh did not know whether or not the spray would <br /> be placed in the pharmacy's hazardous waste tote. <br /> -Four small bags, labeled as"Venlafaxine HCL 75 MG Tablet,""CVS Ibuprofen 200 MG Caplet,""Anastrozole 1 <br /> MG Tablet,"and "Klonopin 0.5 MG Tablet,"each containing loose or broken pills. <br /> -A StrongPak Waste bag containing smaller bags which contained different types of loose pills(IMG 4864). <br /> -A StrongPak Waste bag containing a Boostrix TDAP Vaccine Syringe(IMG 4864). <br /> -A bottle of Oxybutynin 5 MG Syrup, expired 2/1/2019(IMG 4866). <br /> Hanh stated she had not had a chance to place the StrongPak Hazardous Waste and StrongPak Waste bags <br /> containing the loose pills and the Boostrix TDAP Vaccine Syringe, and the bag of used shringrix vials, into the <br /> pharmacy's hazardous waste tote(IMG 4872).According to Hanh, pharmaceuticals placed in the open blue <br /> StrongPak bin are processed once a week, at which point the pharmaceuticals are placed in the tote for Genco <br /> expired returns or the pharmacy's hazardous waste tote. <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. <br /> Immediately close these containers and ensure all hazardous waste containers are closed when not adding or <br /> removing waste. <br /> This is a Class II violation. <br /> FA0023488 PR0541026 SCO01 03/08/2019 <br /> EHD 22-03 Rev.09/22/16 Page 8 of 10 RCRA Large Quantity Hazardous Waste Generator OIR <br />
