The following is an itemized list of underground storage tank violations that have not been addressed for PARKWOODS GAS & FOOD as of January 19, 2022. Open violations from May 18, 2021 inspection Violation#203-Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. According to Jessie Tiwana, Elite IV fixed the leaking dispensers noted in the 2020 inspection report and left a work order. The Elite IV work order was not found on site. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit a missing copy of the work order noted above to the EHD. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#214-Failure to comply with UST operating permit conditions. OBSERVATION:According to Elite IV work order 4861, the diesel product overfill prevention was repaired on 12-5-2018 and a permit was not obtained from the EHD. REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the diesel product overfill prevention.
