The following is an itemized list of underground storage tank violations that have not <br /> been addressed for PARKWOODS GAS & FOOD as of January 19, 2022. <br /> Open violations from May 18, 2021 inspection <br /> Violation#203-Failure to maintain monitoring records for 36 months and maintenance records for 60 <br /> months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. According to <br /> Jessie Tiwana, Elite IV fixed the leaking dispensers noted in the 2020 inspection report and left a work order. The <br /> Elite IV work order was not found on site. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit a missing copy of the work order noted above to the EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#214-Failure to comply with UST operating permit conditions. <br /> OBSERVATION:According to Elite IV work order 4861, the diesel product overfill prevention was repaired on <br /> 12-5-2018 and a permit was not obtained from the EHD. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the diesel product overfill prevention. A <br /> "Failure to Obtain a Permit"penalty applies. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 1 <br />
