i The following is an itemized list of underground storage tank violations that have not been addressed for PARKWOODS GAS & FOOD as of January 19 , 2022 . Open violations from May 18 , 2021 inspection Violation #203 111, Failure to maintain monitoring records for 36 months and maintenance records for 60 PL I months . R ' OBSERVATION : Maintenance and monitoring records for the last three years were not found on site . According to • Jessie Tiwana , Elite IV fixed the leaking dispensers noted In the 2020 Inspection report and left a work order. The ■ T1 Elite IV work order was not found on site. REGULATION GUIDANCE : These records shall be maintained on site for at least three years. Monitoring records r Di include : ( 1 ) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records ; ( 3) results of any visual observations; (4 ) results of sample analysis performed a lab or in the field; (5) logs Hof all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of Sinventory readings and reconciliations . CORRECTIVE ACTION : Immediately locate and maintain all missing maintenance and monitoring records for the Pa last three years on site and submit a missing copy of the work order noted above to the EHD . OBSERVATION : According to Elite IV work order 4861 , the diesel product overfill prevention was repaired on 12-5-2018 and a permit was not obtained from the EHD. ( u, REGULATION GUIDANCE : Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed , trained , and certified contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD . All other repairs must have a permit submitted to and approved by MrE the EHD prior to beginning work . IPS CORRECTIVE ACTION : Immediately obtain a permit from the EHD for the diesel product overfill prevention . A " Failure to Obtain a Permit" penalty applies.
