The following is an itemized list of underground storage tank violations that have not been addressed for A&A GAS & FOOD MART as of Aprill 8 , 2022 . Open violations from February 15 , 2022 inspection Violation #106 - Failure to submit and maintain a current facility plot plan / site map . OBSERVATION An accurate UST Monitoring Site Plan was not submitted . Locations of sensors are not included on the site map . REGULATION GUIDANCE: Asite plan must be submitted identifying the locations where monitoring will be performed. The site plan must show the general layout of tanks and clearly identify locations of the following equipment, if installed : 1 ) monitoring panels ; 2 ) in -tank liquid level probes ( if used for leak detection ) ; 3 ) devices monitoring tank annular spaces or vault; 4 ) devices monitoring product piping ; 5) devices monitoring fill piping ; 6 ) devices monitoring vent piping ; 7 ) devices monitoring vapor recovery piping ; 8 ) devices monitoring vent/transition sumps ; 9 ) devices monitoring under-dispenser containment; 10 ) line leak detectors ; and 11 ) devices monitoring any other secondary containment areas . CORRECTIVE ACTION : Immediately log into the California Environmental Reporting System ( CERS ) at http : //cers . calepa . ca . gov/ and upload a copy of the UST Monitoring Site Plan . ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #109 - Failure to submit " Designated Underground Storage Tank Operator Identification Form" within 30 days . OBSERVATION A new designated operator, Amy Winkler, began conducting inspections on 5/6/2021 and notification was not provided to the EHD within 30 days of the change . REGULATION GUIDANCE: The owner or operator shall submit a " Designated Underground Storage Tank Operator Identification Form " no later than 30 days after any change in the designated UST operator(s) .
