OBSERVATION:An approved copy of the monitoring plans were not available for inspection. REGULATION GUIDANCE:A copy of these plans shall be accessible on site at all times. CORRECTIVE ACTION: Immediately locate and retain a copy on site. This is a repeat violation, Class II. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 3/12/2021, next inspection performed on 5/6/2021. 55 days between the two dates. REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. ----------------------------------------------- OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The owner/operator did not date the 7/1/2021 DO report REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificates dated 1/14/2022 and 1/19/2021 have different'initial training date'&'Date of assuming responsibility as a facility employee'for the same employees. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class II.
