Note that the Urea product observed on-site is a fertilizer, so it's already included in the <br />"Fertilizer" common name on the inventory. Tri -act 1800 was listed as "Cooling Water Treatment"common name on the inventory, so the <br />common name was updated to "Tri -Act 1800 Corrosion Inhibitor". In addition, Lupranate M20 was listed as "Polymethylene Polyphenyl <br />Isocyanate - MDI" common name on the inventory, so the common name was updated to "Lupranate M20". Avail doesn't meet the <br />hazardous criteria, as identified on the Safety Data Sheet, so it was not included on the inventory. The Safety Data Sheets for Avail, Urea, <br />Ashburn Hand Sanitizer, Lupranate M20, and Tri -Act 1800 are attached. <br />Violation #6 - Failed to electronically submit a site map with all required content. <br />� This violation was corrected ❑This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />The 2022 HMBP general site plan was revised and submitted via CERS, which included updating the following items to <br />reflect current site conditions: north orientation arrow, loading areas (gas and diesel), internal site roads, emergency <br />shutoffs (aqueous ammonia and ammonia), emergency equipment (spill kits), and hazardous material handling/storage <br />areas. <br />Violation #10 -Failed to electronically update the HMBP within 30 days of substantial change in operations. <br />❑ This violation was corrected <br />❑ Supporting documents included <br />❑ This violation will be corrected by (date): <br />Describe actions taken or will be taken to correct violation: <br />Simplot will update the HMBP within 30 days of any future substantial change in operations, including any future personnel <br />changes. <br />Page 2 of 3 <br />Rev. 9/16/2020 <br />1868 E.
