The <br /> missing alarms include all alarms from the monitoring system certification testing conducted on 9/7/2021. <br /> REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest <br /> opportunity. <br /> OBSERVATION: The 91 submersible turbine pump(STP)sump sensor was raised approximately 4 inches off the <br /> lowest point of the sump and not located to detect a leak at the earliest opportunity. <br /> REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. <br /> CORRECTIVE ACTION: This was corrected on site. The sensor was relocated to the lowest point of the sump by <br /> the service technician. Ensure that all monitoring equipment is maintained and located to detect a leak at the <br /> earliest opportunity. <br /> This is a repeat violation, Class ll. <br /> 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the 87 submersible turbine pump(STP)sump. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: This was corrected on site. The service technician removed approximately 1/2 a cup of <br /> liquid from the 87 STP sump.
