SAN J O A Q U I N Environmental Health Department - - ` 0UNTY--- Complaint Record Daily Time and Activity Summary from 3/1/2013 to 6/4/2050 for COOSYXA9Z located at 1535 E PESCADERO AVE, TRACY, CA 95304 Activity Date Recorded by 10/14/2021 Received a spill report at 2:10pm for a'dye chloromethane'spill as a result from a PE 2546 PRESTO 180 min fork lift puncturing the 5-gallon pail. SC 544 Called reporting person from hazardous materials reporting company listed on the spill report who provided last name for reporting person from the facility. Called the phone number given and left a voicemail to get more information about the spill for the general operations manager, doc supervisor and shift supervisor.Also called the Environmental Contact who provided a different call person for spills. Left that person a voicemail. Called both emergency contacts in CERS and left voicemails. Spoke with lead senior and it was decided that we should visit the facility. Drove to facility with Shewit Woldearegay.While driving, received a call back from Isaac Mackey, General Operations Manager who said they are in the process of cleaning the spill up and will meet us at the entrance and walk us to the spill. Arrived on site approximately 3:23pm and met with Isaac Mackey. Isaac showed us a hazardous waste drum where they have placed the punctured bucket and residual contents. Per Isaac, approximately 2/3 of the drum was spilled after the bucket was puncture by a forklift unloading the package of buckets. The spill location was outside of the warehouse where the truck trailers load and unload items. The spill had absorbent placed on the spill and was in the process of being swept. There was one 25-gallon drum of debris generated from the spill. Shift Manager, Mark Erdmann stated that they called their hazardous materials company for assistance on how they should clean up the spill and hazards related and equipment needed to clean the spill.The hazardous materials company informed them to use absorbent to clean the spill.The trailer was brought back to the spill area so we could look at the trailer, it did not appear to have residual material inside. Per Isaac,the trailer is being kept away from the other trailers in the meantime to air out.The absorbent sand was adhering to the asphalt and was a gummy/tacky texture.Absorbent sand will be placed on the area to hopefully better absorb the spill overnight.Tomorrow morning the area will be swept up again. There were cones placed around the spill and absorbent to secure the area.A copy of the shipping slip for the punctured shipment was provided and a copy of the SDS. 10/15/2021 Created complaint. Generated Prop65 for mailing. Placed Prop65 in mailing bin. PE 2546 PRESTO 156 min Saved all photos into shared drive. SC 007 10/15/2021 Generated Notice to Abate. Emailed to Isaac Mackey, General Operations PE 2546 PRESTO 72 min Manager, isaac.mackey@yrcfreight.com SC 544 Spoke with Isaac who stated that they called in their hazardous materials contractor to clean up to spill as there was still absorbent stuck to the asphalt after leaving the absorbent overnight. Requested photos from Isaac showing the final clean up. Noticed that their SDS received on site yesterday from their environmental person was for benzothiazole but the shipping slip says dichloromethane. Found a SDS online for dichloromethane. Clean up method for dichloromethane is take up with liquid-absorbent materials and for benothiazole is with inert absorbent material. Page 1 of 2
