<br />Environmental Health Department <br /> <br />SAN JOAQUIN <br />--COUN f Y---- <br />Greatness grows here Time In: <br /> <br />Time Out <br />WASTE DISCHARGE/SPILL RESPONSE COMPLAINT INSPECTION <br />REPORT <br />Name of Facility: WASTE MANAGEMENT Date 12/10/2020 <br />Address: 11303 N 99 FRONTAGE RD, LODI <br />Owner/Operator TWIN OAKS MOBILE HOME PARK LLC Telephone: <br />Program Element: 2546- RELEASE/SPILL RESPONSE (EXCLUDING JOINT TEAM) Complaint #: C00053109 <br />Responsible Party: HENRY FONTES (CONTACT) <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />Two gallons of hydraulic oil spilled onto asphalt approximately 120 feet west of 11303 99 Frontage Rd. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />Received a Cal OES Spill Report Control #: 20-6830. Henry Fontes (Route Manager at Waste Management) reported a <br />two-gallon spill of hydraulic oil. This was due to the hydraulic hose becoming disconnected from the line when struck by a <br />piece of wood. Driver shut off the power take-off (PTO) pump and and contained the hydraulic oil spill. Driver applied <br />absorbent and cleaned the impacted asphalt. No waterways were impacted. Approximately one-third partially full 35-gallon <br />container of absorbent was generated from the cleanup and was taken to the 1333 E. Turner Rd. site pending disposal. <br />On site, located the spill area adjacent to three green Waste Management dumpsters. Observed the spill area was cleaned <br />with absorbent. The area covered approximately 25 x 25 feet. No oily residue or odor was observed. Site was adequately <br />cleaned. <br />Provide proof the failed hydraulic hose was repaired and a copy of the final disposal record for the waste generated from <br />cleanup. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
