The 24 hours phone number(510)224-7321 for the primary emergency contact listed under the <br /> Business Owner/Operator Identification form is incorrect. <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATIONS: <br /> A- The followings are Incomplete/inaccurate chemical inventory information: <br /> 1. Unknown amount of transformer oil for the electric transformers have not been submitted to CERS. <br /> 2. Facility has three 55 gallons drums of motor oil and only reported one 55 gallons drum to CERS. <br /> 3. Facility has two tanks of Sodium Bisulfite with maximum container size of 6,800 gallons each, and only <br /> one tank reported to CERS. <br /> ------------------------------------------------------------------------------------------------------------------------------- <br /> B-The chemical inventory forms for the following chemicals are not complete <br /> 1. Motor Oil: Missing storage pressure and temperature <br /> 2. Used Oil: Missing storage pressure and temperature <br /> 3. Nitrogen: Missing storage pressure and temperature <br /> 4. Pac Polymer: Missing Storage Pressure <br /> 5. Caustic Soda Liquid 50%: Missing Storage Pressure <br /> 6. Sodium Hydroxide: Hazardous Material type and Storage Pressure <br /> 7.
