S0 JOA U 1 Environmental Health Department <br /> U ;� I YI AMENDED <br /> 2/10/2022 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CITY OF STOCKTON TERTIARY TREATMENT 3307 W HWY 4 , STOCKTON February 08, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. The 2020 submittal received late <br /> on 2/12/2020 and the 2019 submittal received late on 1/17/2019. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year <br /> CORRECTIVE ACTION: Submit a statement to SJC EHD stating that the business will annually certify/submit the <br /> annual HMBP to CERS by January 15. <br /> This is a minor violation. <br /> 10 HSC 25508.1(a)-(f) Failed to electronically update the HMBP within 30 days of substantial change in operations. <br /> OBSERVATION: The Business Owner/Operator Identification form was not submitted or complete with all accurate <br /> information. John Walker( Primary Emergency Contact)stopped working for the business 2 years ago and the <br /> Business Owner/Operator Identification form was not updated within 30 days.The Business Owner/Operator <br /> Identification form was updated on 2/1/2022. <br /> REGULATION GUIDANCE: Within 30 days of one the following events, business shall electronically update the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> a) A 100 percent or more increase in the quantity of a previously disclosed material. <br /> b) Any handling of a previously undisclosed hazardous materials <br /> c) A change of business or facility address <br /> d)Change business ownership <br /> e)Change of business name <br /> f)(1)A substantial change in the handler's operations occurs that requires modification to any portion of the <br /> business plan. <br /> (2)For the purpose of this subdivision, "substantial change" means any change in a facility that would inhibit <br /> immediate response during an emergency by either site personnel or emergency response personnel, or that could <br /> inhibit the handler's ability to comply with Section 25507 , change the operational knowledge of the facility, <br /> or impede implementation of the business plan. <br /> Violation was corrected on 2/1/2021. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended <br /> and replaces the initial report.
