Goodwill Industries of SJV <br /> CERS ID:10779607 <br /> Inspection Date: January 27, 2022 <br /> Corrective Action Statement for violation #102, #605, #704, #708, #712 <br /> Violation#102—Failed to determine if a waste is hazardous waste. <br /> Correction Statement: <br /> Medication look-up will be used to determine hazardous waste determination,training manual has been <br /> updated. All waste medications will need to be identified as controlled or non-controlled. They will be stored <br /> separately in storage bin with content label indicating that its Hazardous Waste, contents reflecting controlled <br /> pharmaceuticals or non-controlled pharmaceuticals, accumulation date, and site address. <br /> All non-controlled pharmaceutical waste will be disposed of each month by reporting it to our Clean Earth HW <br /> disposal representative to arrange a date for disposal at San Joaquin County disposal site. <br /> All controlled pharmaceuticals will be stored separately and taken to a drop-off site. Reached out to Stericycle <br /> for Seal & Send Mail Back for medication program. <br /> Resolution: <br /> What we did to fix this violation with waste medications, we called our local Environmental Disposal site and <br /> asked if they disposed of unwanted/waste medications. They indicated that they take only non-controlled <br /> substances. For all medications on hand, using SDS we were able to determine if controlled or uncontrolled. <br /> Submitted the completed CESGQ Inv. List to our HW disposal rep. From Clean Earth. She was able assist us by <br /> adding to waste check-in list, they were packaged (all personal identification blacked out and all properly <br /> bagged. Disposed on 02/23/22 @ San Joaquin County 7850 R.A Bridgeford ST. Stockton. (See attached <br /> certification statement page 1). <br /> We did have 1 controlled waste medication—Promethazine w/Codeine Syrup. It was the only'controlled' <br /> medication on-hand as well as 8 new unopened Enoxaparin injection. Since they still had medication inside vial, <br /> they were not able to be sent to local waste disposal site, nor were they able to be placed in a sharp container <br /> due to having medication still in viral,they were included and taken to a drop-off site. Site was found by looking <br /> https://www.dca.ca.gov/webapps/pharmacy/takeback search.php I located site RX Express Pharmacy 711 E. Market ST. Stockton <br /> that has a drug collection bin. I prepared the items by scratching off the personal information and securing it <br /> back in a transparent plastic bag and taking it to pharmacy. <br />
