7) Check that all testing and maintenance has been completed and documented. 8) Verify that all facility employees have been trained in accordance with 23 CCR 2715(c). 9) The owner/operator sign and date the Designated Underground Storage Tank Operator Visual Inspection Report within 72 hours of being provided a signed copy. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: - The DO reports of 11/1/2021, 9/3/2021, 8/6/2021, 5/22/2022, 4/22/22, 3/25/2022, 2/25/2022 and 1/28/2022 did not include the owner/operator name, signature and date signed. -The DO reports of 12/31/2021, 12/01/2021, 10/03/2021, 07/08/2021 and 06/09/2021 did not include the owner/operator name. -The DO report of 11/01/2021 did not include follow up actions in section 5 to compliance issues from section 3 REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Corrective Action Ensure that designated operators performing visual inspections at this facility are including all the required information on the reports Response RTC form was signed and submitted 6/02/2022 — see attachments Fast Easy Mart 103 RTC & corrective action 060222 (page 3) and Fast Easy Mart 103 DO visual inspection report 060921-052222. Designated operator has been instructed to fully complete all required information on the reports, and facility operator will check and sign reports going forward. Violation #214 Failure to comply with any of the applicable requirements of the permit issued for the operation of the UST system. OBSERVATION: The facility is not in compliance with the written conditions of the operating permit. The overfill prevention equipment testing due by 10/2021 has not been conducted. REGULATION GUIDANCE: The owner and operator of underground storage tank (UST) systems shall comply with all requirements of the permit issued for the operation of the UST system. Corrective Action Immediately take all necessary actions to bring these UST systems into compliance. Schedule for overfill prevention equipment testing now. Response RTC form was signed and submitted 6/02/2022 — see attachments Fast Easy Mart 103 RTC & corrective action 060222 (page 4) and Fast Easy Mart Stockton Full OPEI Report 2021-10-19. Next inspection is due by 10/19/2024 and will be conducted prior to that date with OPEI Report forwarded to CUPA when received. Violation #209 Failure to maintain a copy of the "Facility Employee Training Certificate" on-site or off-site at a readily available location, if approved by the UPA. Corrective Action Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD.
