S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> THORNTON 76 8606 THORNTON RD, STOCKTON March 21, 2022 <br /> Other Violations <br /> 2010 See below Unlisted General violation ❑V ❑R ❑COS <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 102 CCR 2711 Failure to submit a complete and accurate application for a permit to operate a UST. <br /> OBSERVATION: The UST Facility Operating Permit Application is lacking the California Department of Tax and Fee <br /> Administration (CDTFA) Underground Storage Tank Account Number. <br /> REGULATION GUIDANCE: The application for a permit to operate an underground storage tank shall include the <br /> CDTFA registration number issued to the owner of the tank, in accordance with HSC 25286. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS), update the <br /> required information, and submit for review by the EHD. <br /> This is a minor violation. <br /> 106 CCR 2632(d), 2641(h) Failure to submit and maintain a current facility plot plan/site map. <br /> OBSERVATION:An accurate UST Monitoring Site Plan was not submitted. <br /> -E85 tank is not in between the two tanks, it should be on the northwest side of the facility <br /> -Premium tank is not on the northwest side of the facility, it should be in between the two UST <br /> REGULATION GUIDANCE:A site plan must be submitted identifying the locations where monitoring will be <br /> performed. The site plan must show the general layout of tanks and clearly identify locations of the following <br /> equipment, if installed: 1)monitoring panels; 2)in-tank liquid level probes(if used for leak detection); 3)devices <br /> monitoring tank annular spaces or vault; 4)devices monitoring product piping; 5)devices monitoring fill piping; 6) <br /> devices monitoring vent piping; 7)devices monitoring vapor recovery piping; 8)devices monitoring vent/transition <br /> sumps; 9)devices monitoring under-dispenser containment; 10)line leak detectors; and 11)devices monitoring any <br /> other secondary containment areas. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class II. <br /> 109 CCR 2715(a)(1)(B),2715(a)(3) Failure to submit"Designated Underground Storage Tank Operator Identification <br /> Form"within 30 days. <br /> OBSERVATION:A new designated operator,Amy Winkler, began conducting inspections on 6/3/2021 and <br /> notification was not provided to the EHD within 30 days of the change. <br /> REGULATION GUIDANCE: The owner or operator shall submit a"Designated Underground Storage Tank Operator <br /> Identification Form"no later than 30 days after any change in the designated UST operator(s). <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> This is a minor violation. <br /> FA0002590 PR0232261 SCO01 03/21/2022 <br /> EHD 23-01 Rev.12/06/2021 Page 4 of 5 Underground Storage Tank Program OIR <br /> 1868 E.
