SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> 7-ELEVEN INC#41341 14405 PACIFIC AVE, STOCKTON June 02, 2022 <br /> Other Violations <br /> 2010 See below Unlisted General violation ❑V ❑R ❑COS <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 101 HSC 25284; CCR 2711(d) Failure to obtain a permit to operate from the EHD including but not limited to unpaid <br /> permit fees. <br /> OBSERVATION: The facility operated without a permit from 1/1/2022-2/9/2022 due to unpaid permit fees. <br /> REGULATION GUIDANCE: No person may own or operate an UST unless a permit for its operation has been <br /> issued by the local agency to the owner or operator of the UST system. <br /> CORRECTIVE ACTION: Ensure that permit fees are paid on time in order to obtain a permit to operate ethe UST <br /> system. Provide a corrective action statement to the EHD. <br /> This is a Class I violation. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. Per the <br /> 6/28/2021 designated operator report, liquid was observed in UDC 1/2. The notes indicated a technician was <br /> dispatched to get the liquid (Ticket#INC3329015). Per the 7/23/2021 designated operator report, no liquid was <br /> observed in UDC 1/2. The work order was not found during the inspection. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit the missing copies to the EHD. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 2, 2022. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
