Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />March 10, 2022 <br /> Facility Address: <br /> 5100 W HWY 12 , LODI <br /> Facility Name: <br /> FRANK C ALEGRE TRUCKING INC <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: One diesel storage tank (estimated to be able to store 8000 gallons) was found on site that have <br />not been reported to CERS. Mr. Henry Rotor indicated that this tank is used for overflow fuel deliveries and has <br />been brought to the site sometime in the last 1-2 years. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br />approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: An adequate site map was not submitted as part of the business plan. 1) Several features on the <br />submitted map are illegible, for example label for the tank located adjacent to the safety office is not readable along <br />with some other site features. 2) No storm or sewer drains were included on the submitted map. 3) One diesel <br />storage tank (estimated to be able to store 8000 gallons) was found on site that have not been included on the site <br />map. Mr. Henry Rotor indicated that this tank is used for overflow fuel deliveries and has been brought to the site <br />sometime in the last 1-2 years. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the corrected and legible site map that includes all the required site map <br />elements. Submit proof of correction to the EHD. <br />This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15.
