Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />March 08, 2022 <br /> Facility Address: <br /> 1131 HOLLY DR, TRACY <br /> Facility Name: <br /> TIRE CHOICE <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. <br />The following were missing from the site map: <br />- loading area <br />- internal roads <br />- oil room adjacent to the tire storage was not shown on the map <br />- the spill kit in the oil room was not listed <br />- the 55-gallon drum of motor oil was not listed in the oil room <br />- fire extinguisher in the middle of the repair area <br />- nitrogen in the middle of the repair area <br />The following were listed on the map but was not observed on site: <br />- the second fire extinguisher in the hazardous waste storage area was not observed during the inspection <br />- the fire extinguisher inside of the tire storage was not observed on site <br />- the spill kit inside of the tire storage was not observed on site <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. <br />- The 2022 business plan submittal was submitted late on 1/19/2022 (facility, plans) and 2/25/2022 (inventory) <br />- The 2021 business plan submittal was submitted late on 2/5/2021 <br />- The 2020 business plan submittal was submitted late on 2/13/2020 <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Ensure business plan submittals (facility information, hazardous materials inventory, <br />emergency response and training plans) are submitted annually by January 15 by logging into the CERS at <br />http://cers.calepa.ca.gov/, entering the correct or updated information, and submitting to the EHD for approval. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were two violation(s) during the <br />inspection.
