SAN J OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: TateULLOAS TOW &AUTO REPAIR 620 S WILSON WAY, STOCKTON ch 18, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 1 SJO 4432 Failed to pay annual operating fees. <br /> OBSERVATION: Facility failed to pay annual operating fees for the year for 2022. <br /> ORDINANCE: Fees shall be paid once a year at the time of submittal of an inventory form. <br /> CORRECTIVE ACTION: Immediately pay the annual fee to the EHD Accounting Office located at 1868 E Hazelton <br /> Avenue, Stockton, CA 95205. Fee may also be paid online at https://www.sjgov.org/department/envhealth/fees <br /> (Convenience fee may apply). <br /> This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS) annually by the due date, January 15. The business plan information was <br /> not submitted for 2020, 2021, and 2022. The business plan information was last submitted on 12/27/2018. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a repeat violation, Class 11. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 19, 2022. <br /> Assisted with CERS access and submittal at the time of inspection. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
