SAN JO,AQUIN <br /> Environmental Health Department <br />Time In: <br />Time Out: <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: CITY ROAD - RESIDENTIAL AREA Date: 02/22/2022 <br />Address: 2645 PETAL WAY, LODI <br />Owner/Operator: CITY OF LODI Telephone: (209) 333-6706 <br />Program Element: 2546 - RELEASE/SPILL RESPONSE (EXCLUDING JOINT TEAM) Complaint #: C00054702 <br />Responsible Party Contact: HENRY FONTES <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />APPROXIMATELY 2 GALLONS OF COOLANT WAS RELEASED FROM A WASTE MANAGEMENT TRUCK ONTO THE <br />ROAD/ASPHALT <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />The Environmental Health Department received notification of an approximate 2 gallon release of coolant. The coolant <br />released from a waste management truck onto the road/asphalt due to a damaged hose. I contacted Henry Fontes (Route <br />Manager of Central Valley Waste Services) who confirmed the release. There were no waterways impacted, no exposures, <br />and no evacuations. Per Henry, the truck was driving west on Petal Way. When the truck stopped in front of 2645 Petal <br />Way, the driver discovered the release. The release impacted the road/asphalt where the truck stopped and impacted Petal <br />Way east of where the truck stopped. Absorbent was applied to the impacted areas, containerized, and taken to 1333 E <br />Turner Rd, Lodi, CA 95240 where it would be stored until disposal. Per Henry, the waste will be disposed of as hazardous <br />waste. Per Henry, the release has been stopped and all impacted areas have been cleaned. Photos of the clean-up were <br />received from Henry and clean-up has been verified. <br />Please be advised that the disposal of any waste [HSC 5411] or disposal of any hazardous waste, or the causing thereof, is <br />prohibited when the disposal is at a facility that is not permitted or authorized for hazardous waste disposal. [HSC <br />25189.5(a).]. Please be aware that all hazardous waste shall be disposed of under appropriate hazardous waste manifesting <br />procedure [HSC 25160 (b)(3)]. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
