Date 08/14/2021 <br />Address: H\NY 4 & HEWITT RD, FARMINGTON <br />Owner/Operator: Telephone: <br />Program Element: 2546- RELEASE/SPILL RESPONSE (EXCLUDING JOINT TEAM) Complaint #: COIS4DASJ <br />Responsible Party Contact: ALEX STEELE <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />CAL OES SPILL REPORT #21-4378: OVERHEAD TRANSFORMER POLE STRUCK BY VEHICLE RESULTING IN 35 <br />GALLON RELEASE OF MINERAL OIL (NON PCB) ONTO SOIL ONLY. NO WATERWAYS IMPACTED. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />Called and spoke to Brian Hoffman of PG&E, who reported the spill to Cal OES, and who stated Dave Larsen notified him at <br />approximately 5:40 pm that the spill had occurred, the clean-up was complete, and the excavated soil was in a roll-off bin <br />on-site. Brian stated the transformer that spilled was placarded as containing Non PCB oil. Called and left a voice message <br />for Dave Larsen to verify an oil sample from the transformer will be analyzed for PCBs. EHD to visit the site on 8/15/2021 to <br />verify clean-up is complete. <br />Please be advised that the disposal of any waste [HSC 5411] or disposal of any hazardous waste, or the causing thereof, is <br />prohibited when the disposal is at a facility which is not permitted or authorized for hazardous waste disposal. [HSC <br />25189.5(a).]. Please be aware that all hazardous waste shall be disposed of under appropriate hazardous waste manifesting <br />procedure [HSC 25160 (b)(3)]. <br />Provide all hazardous waste determinations, oil sample PCB analytical results, and waste/hazardous waste disposal records <br />to the EHD for all clean up debris associated with this clean up, within 30 days. If the PCB analytical results show that the <br />transformer oil contains PCBs, further clean-up and confirmation sampling is required per 40 CFR 761.125. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
